Certification of Births and Deaths
In Palau, birth and death records are prepared through the Medical Records Unit at Belau National Hospital (BNH). Applications and supporting documents are submitted to Medical Records for verification and processing. Once the record is processed and filed with the Civil Registrar, applicants may request certified legal copies from the Clerk of Courts at the Palau Supreme Court.
Birth Certificate
Parents should complete the Birth Certificate Application Form and submit it to the Medical Records Unit within 10 days of the child’s birth. Once all required signatures have been obtained, the Certificate is filed with the Clerk of Courts.
Death Certificate
An informant or close family member of the deceased should schedule an appointment with the Medical Records Unit to review the Certificate of Death Form for accuracy and submit the required supporting documents. Once all required signatures have been obtained, the Certificate is filed with the Civil Registrar, and applicants may request certified legal copies from the Clerk of Courts at the Palau Supreme Court.
